Requests for Student Transcripts

Former Students

Requests for transcripts for former students need to be made to the following attended/graduated High School or Program with the following information:

  • Full name of student when enrolled
  • Date of birth
  • Dates of attendance
  • Email and/or mailing address to send the transcript
